O MAIOR GRUPO DE CRIANÇAS COM BAIXA ESTATURA LINEAR INCLUINDO INFANTIL, JUVENIL E MESMO ADOLESCENTE APRESENTA CAUSA DESCONHECIDA E SITUA-SE NUMA FAIXA CONSIDERADA COM VARIANTE DO CRESCIMENTO NORMAL, ESTÃO ABAIXO DO 3º PERCENTIL DE ALTURA E APRESENTAM BAIXA VELOCIDADE DE CRESCIMENTO, GERALMENTE COM RETARDO NA MATURAÇÃO DO ESQUELETO, E CRESCIMENTO MENOR QUE O ESPERADO NA FASE PUBERAL. FISIOLOGIA–ENDOCRINOLOGIA–NEUROENDOCRINOLOGIA–GENÉTICA–ENDÓCRINO-PEDIATRIA (SUBDIVISÃO DA ENDOCRINOLOGIA): DR. JOÃO SANTOS CAIO JR. ET DRA. HENRIQUETA VERLANGIERI CAIO.

IDIOPATHIC LOW HEIGHT, CONSTITUTIONAL DELAY OF GROWTH AND PUBERTY, LOW HEIGHT OR GENETIC FAMILY.

THE LARGEST GROUP OF CHILDREN WITH LINEAR LOW HEIGHT INCLUDING CHILD, YOUTH AND EVEN CAUSE UNKNOWN PRESENTS TEEN LOCATED IN A BAND WITH ALTERNATIVE CONSIDERED NORMAL GROWTH ARE DOWN 3RD PERCENTILE OF HEIGHT AND PRESENT LOW RATE OF GROWTH, USUALLY WITH THE DELAY OF MATURITY SKELETON, AND LESS THAN EXPECTED GROWTH PUBERTAL PHASE. NEUROENDOCRINOLOGY PHYSIOLOGY-ENDOCRINOLOGY-GENETICS-ENDOCRINE-PEDIATRICS (SUBDIVISION OF ENDOCRINOLOGY): DR. JOÃO SANTOS CAIO JR. ET DRA. HENRIQUETA VERLANGIERI CAIO.

These youth, infant, child and adolescent do not have any chronic or endocrine diseases, have normal secretion of GH and family history may or may not present with these clinical characteristics, but exhibit short stature (height). Among the more frequent we will highlight some of which must be observed very carefully.

CONSTITUTIONAL DELAY OF GROWTH AND PUBERTY:

Constitutional delay of growth and puberty occurs in children with childhood stunting, delayed puberty, delayed bone age and delayed pubertal growth reaching height within the normal range in adulthood. Although the most common disorders of delayed puberty are idiopathic, the concerns raised by delayed puberty are many and variously appreciated by different people. These include the possibility of underlying pathology, fear that puberty never emotional disturbance and psychosocial immaturity, especially short stature and associated occur. Furthermore, concerns about long-term sequelae which comprise reducing the final adult height (Ht) - terminal high, mineralization and bone fertility can also be raised. Pattern of inheritance of constitutional delay of growth and puberty (CDGP) is consistent with autosomal dominant inheritance. It occurs twice as often in boys against girls. Ht terminal height and weight and the short terminal Ht height shortfall being are usually evident in the presentation and continue until terminal adult height Ht end. The age of onset of puberty is delayed by an average of 2.5 years for girls and 3 years for boys. Ht terminal deficit at the beginning of puberty height correlates with height Ht terminal end. The time between the onset of puberty and pubertal growth spurt is smaller than in normal juvenile and adolescent children's children and the growth rate peak is attenuated. Therefore, the final average Ht is usually shorter than both target Ht terminal height and predicted adult height terminal.

Resultado de imagem para CONSTITUTIONAL DELAY OF GROWTH AND PUBERTY:
The height Ht terminal end is on average 1.85 SD below the mean for healthy adults, large individual variations. Best Ht terminal end point, seems to occur infant children youth and teens who had their slow growth during his late childhood, while those with early and progressive decrease in the height Ht terminal end in early childhood have committed Ht end terminal height. Moreover, constitutional delay of growth and puberty (CDGP) is combined with familial short stature around 40% of children with short height Ht terminal end <3rd percentile for age and sex. The secretion of GH - growth hormone may be transiently decreased at the time of delayed pubertal growth, probably due to decreased gonadal steroid production.

LOW HEIGHT OR FAMILY GENETICS:

The height and the growth pattern of the family determines the growth of the individual, so that a child growing up with different siblings 'and parents' standard should be evaluated clinically. Genetic transmission of organic diseases may lead to short stature (height). Insensitivity to GH - growth hormone caused by mutation of the GH receptor gene - growth hormone, GH gene deletions, mutations of PROP-1 gene, pseudo-hypoparathyroidism, diabetes mellitus, some forms of hypothyroidism, Osteochondrodysplasia, inborn errors of metabolism, renal disease and thalassemia are in hereditary abnormalities that can induce short stature (height) linear. Low genetic or familial stature is in a different group from that described, with the clinical characteristics growth in childhood below the 5th percentile but with normal speed. Puberty normally occurs, or slightly earlier, and faster than normal, with bone and chronological ages in agreement, but that led to a linear low stature. Parents are low and the final height of patients is low, situated on Family Band, with no abnormality of the GH-IGF axis, but one should take into account that the correction of stature for parents, possibly due to gold standard therapy be relatively new to everyone, injunctions may have occurred that did not allow parents to have the ideal target or according to ancestral little newer generations chronologically default, must be at least a subjective perception and not think of Cartesian form.
